The Physical Education waiver information is now available by clicking here. The Physical Education Application form is now available by clicking here. Both are also available in the Athletic Office. Current sophomores and juniors may apply for this waiver FOR NEXT YEAR if they will be participating in a MFHS lettering sport (some restrictions). The purpose is to enable student athletes to enroll in an additional (higher level) academic class. The form absolutely must be turned in to the Athletic Office with all pertinent signatures (student, parents, varsity head coach) by March 5, 2021.No late forms will be accepted. If your student had their picture taken this fall or winter with their athletic team, his/her proof is now ready for viewing/ordering. In order to access, you will need to register with Empire Photography using the link below. Once you have registered, Empire will contact you regarding your sports photo. The yearbook staff invites parents and students to submit photos for the yearbook by clicking on the URL below. If you’ve got some good photos of school events or of students and their friends just having fun, we’d like to see them! Our photographers can’t be everywhere. Help us get as many photos as possible to create a terrific yearbook everyone will want to have. Please send jpgs only, not png’s, tiff’s or HEIC photos. Thank you!

Sports registration for the 2020-21 school year is open for all sports. Athletes need to have a valid (dated April 1, 2019 or after) physical to be approved.
If you would like to check to see what we have on file for your physical, click here to log in to your account. Then click on the edit link under the Physical column. This will show you a list of physicals that have been uploaded for that student that you can click on.

This service is not endorsed by SDMF, but is an option for a sports physical if your student needs one right away and you cannot get in to your regular primary medical provider.
Minute Clinic (located inside the CVS) offers sport physicals every day. For this service, they don’t accept insurance, but sports physicals are just $49 right now! Students under age 18 should be accompanied by a parent or guardian. Families are encouraged to bring their student’s vaccination record for the nurse practitioner to review.
